Make an appointment
The best way to make an appointment is to send an email to video@horaczek.de.
To make an appointment, we need your
- Surname, first name
- Date of birth
- Health insurance provider
- Mobile phone number

Once your appointment has been confirmed, you will receive a personal login code (TAN). Please log in here with your code at least 5 minutes before the agreed time: Login video consultation.
We can send you prescriptions and referrals after the video consultation. To do this, you should provide postage or, if possible, a stamped envelope at the latest by your next appointment.
Your health insurance card must be scanned once per quarter.
This can be sent to us by post (please enclose a stamped envelope) or must be scanned at your next in-person appointment.

Technical requirements
The video consultation works without an app, directly via the Jameda website in your browser. You can participate in the video consultations from any device, whether it be a smartphone or laptop. To ensure that everything runs smoothly, all you need is a stable internet connection, a camera, a microphone and speakers.
